Media Festival Awards Entry Form

The 40th Annual MCA-I Media Festival

40 Years of celebrating the best in our industry

Instructions:

  • Read the Media Festival Entry Rules before attempting to fill out this form online. It details all the Categories (General, Craft and Student)
  • Complete the entire Entry Form.
  • Make only one media entry per Entry Form.
  • Entry forms must be submitted by Friday June 13th, 2008.
  • Select one of the Professional Categories OR the Student program category. Student entries will be judged with other student entries. (Students may enter as Professionals, but must pay the Professional fee.)
  • Payment using the Online Form is by Credit Card only. To pay by check, do not use this online form, instead, print and fill out this PDF form to send in with your check and your entry.
  • If you are paying by Credit Card and wish to use this Online Form, you will be asked to submit a filled out copy of this form along with your DVD entry (one per entry). This is to be sure that we match up your mailed DVD entry with the Online Form. You do not need to do this if your entry is a URL. (To save you time - please PRINT THIS PAGE - prior to hitting the Submit button. If you do not do this, you will receive a Confirmation notice with a link to the PDF form which you will need to fill out to send with your DVD entry. We have asked programming to add a "Print this Form" option to the Confirmation process, but it is not active yet.)
