43rd Annual MCA-I Media Festival Entry Form

 

Celebrating the best in our industry (Please come back after the Call for Entries for the 44th Annual MCA-I Media Festival has been announced.)

Instructions:

  • Read the Media Festival Entry Rules before filling out this online form. It details all the Categories.
  • Complete the entire Entry Form.
  • Make only one media entry per Entry Form.
  • A program can be judged under more than one category (e.g.m, documentaries and writing). Each category requires a separate entry form and entry fee.
  • Entry forms must be submitted by midnight (Central time) Wednesday, Novermber 30, 2011.
  • Select one of the Professional Categories OR the Student program category. Student entries will be judged with other student entries. (Students may enter as Professionals, but must pay the Professional fee.)
  • Payment using the Online Form is by Credit Card only. 
  • You must print a copy of this form and send it along with your DVD entry (one per entry). This is to be sure that we match up your mailed DVD entry with the Online Form (To save you time - please PRINT THIS PAGE - prior to hitting the Submit button.
  • Packages with DVDs must be postmarked no later than December 9, 2011.

Fees are as follows. Please select the appropriate dollar amount from the drop down list below. Entry fees must be paid in U.S. funds. Each Entry is on a separate form.

Member ($80 per entry)
Non-member ($110 per entry)
Student Member ($25 per entry)
Student Non-member ($40 per entry)
